Adam Mychajliw, Samuel Case Obituary, Death: Valley Teen Among Five Killed in I-40 Multi-Vehicle Crash

Adam Mychajliw, Samuel Case Obituary, Death – A tragic multi-vehicle crash on Interstate 40 near Holbrook, Arizona, claimed the lives of a Valley teen and four adults, according to officials. The Arizona Department of Public Safety (DPS) responded to the scene on Friday, March 14, at milepost 294 on the westbound lanes of I-40.
Preliminary investigations revealed that the crash was triggered when a semi-truck rear-ended a Cadillac Escalade, setting off a chain reaction involving a Jeep Wrangler and another semi-truck. As a result, five individuals lost their lives, and two others sustained injuries.
Authorities identified the deceased victims as:
- Adam Mychajliw, 17, of Scottsdale, Arizona
- Sophia Case, 17, of Fort Collins, Colorado
- Sarah Broyel-Case, 50, of Fort Collins, Colorado
- Samuel Case, 51, of Fort Collins, Colorado
- Jeet Vikram, 34, of Ozone Park, New York
DPS officials noted that the crash occurred amid heavy traffic congestion, which had resulted from detours caused by winter storm-related road closures in Arizona’s high country.
The investigation into the crash is ongoing.
